Goal: The goal of the study was to evaluate the association between Medicaid‐provided nonemergency medical transportation and diabetes care visits.

Impact: The findings of this study underscore the importance of ensuring transportation to Medicaid populations with diabetes, particularly in the rural areas where the prevalence of diabetes and complications are higher and the availability of medical resources lower than in the urban areas.

Goal: The goal of the Place Matters initiative is to improve the health of participating communities by “addressing social conditions that lead to poor health” through “identifying the complex root causes of health disparities and defining strategies to address them.”

Impact: AC Place Matters has identified 10 best practices that could help other programs address social determinants of health: find and foster strong leadership, dedicate staff resources to the work, engage staff from across the local health department, contribute to building grassroots power, address root causes, partner with community organizations and leaders, partner with government institutions across sectors, work reactively and proactively, build capacity, and use tools that ensure a focus on health equity.

Goal: The goal of Baby, Be Safe is to increase the use of child injury prevention measures.

Impact: Participants who received tailored educational materials reported greater adoption of home and car safety behaviors than those receiving generic information. This study offers promising findings to help prevent injuries to young children.

Goal: The goal of the Baltimore Needle Exchange Program is to reduce HIV, hepatitis, and other infections by reducing the use of unclean needles and to help individuals overcome substance abuse by connecting them to harm reduction services and drug treatment programs. The experimental case manager intervention program at the Baltimore NEP looked to increase the percentage of intravenous drug users who enrolled in city sponsored substance abuse programs following referral at the Baltimore NEP sites.

Impact: The intervention program through Baltimore NEP was effective in increasing entry of intravenous drug users into drug drug treatment programs and highlights the need for more accessible treatment programs and harm-reduction services, such as mobile treatment facilities.

Goal: The purpose of California Main Street is to educate and provide assistance to California communities about how they can revitalize their downtowns or core commercial districts using the self-help Main Street Four-Point Approach. Through this education on how traditional downtowns and neighborhood commercial districts can become and remain vital economic, social, and cultural centers, the community's development leads to livable and sustainable practices that ultimately improve its quality of life.
